Fill out a UW Human subjects form, along with a consent for one of the studies presented in a reading to the class. Write the human subject form as if you are a University of Washinton researcher wanting to conduct the study at UW.

Submit one copy of the form UW 13-11 and a copy of your consent form to Alan.
