Turn in an IRB application. There are multiple versions of this form because UW is in the process of transitioning between different ways of submitting them. You may use any of them.
Note that the IRB application requires you to submit all study materials, in the form you will present them to users. This includes your recruiting materials, a script of what you will say or show to them during the experiment, any forms you will ask them to fill out, and so forth. Don't omit this.
